FCHS is offering reduced-cost lab work as part of our "Wellness Wednesday" program the 3rd Wednesday of every month. FCHS will not collect health insurance information and no claim will be sent to insurance for processing. Payment is due at the time of service.

Health Screening Profile - $40.00
(Includes CMP, Hemogram, Lipid Panel, TSH)

PSA (Prostate) - $25.00
HA1C (Diabetes) - $25.00
FT4 (Thyroid) - $20.00

A $3 discount will be offered to Seniors 65 years old or older after 8 a.m. Screening requires patients to fast for 10 to 12 hours before. Refreshments will be provided!
